Data Request Form

This form is used for obtaining data that cannot be obtained through the current online PHR Reports. Please peruse the PHR Reports prior to making a data request to ensure the data is not available in an existing report. If the data is not available, please complete the form and submit it. Once the form is submitted, it is received by UHR Information Services and you will be contacted if there are any questions regarding your request. The expected turnaround time is within 3-5 days depending on the complexity of the request.
